If the notice of the order of suspension has not been served, the department will mail a written notice of the order of the rest to the last known address.

The order of suspension will specify the statutory grounds for the rest – the effective date – and the right of the person to request an administrative hearing, along with the procedure for requesting an administrative hearing.
